Analysis on problems existed in nursing records under various nursing modes
Huli yanjiu, 2004 对我国不同护理模式下护理记录存在问题进行分析 ,认为功能制护理记录过于简单 ,护士协助医生为主要任务 ,不能体现护理价值 ;整体护理记录较繁琐 ,记录量大 ,表格病历的证据效力不足 ,此二者均不利于举证。同时认为 ,护理程序思维并不过时 ,应将护理程序 5步骤贯穿于护理记录之中 ,使之更具客观性、科学性、证据性。此外 ,对《病历书写规范》要求的护理记录从证据意识、主客观资料的判断、应该记录的频次及记录范围 4个方面进行了阐释 ,试图达到提高护士正确、规范书写护理记录的目的。
