Absent Right Superior Vena Cava and Persistent Left Superior Vena Cava: The Perioperative Implications

A 22-yr-old female patient was admitted for repair of tetralogy of Fallot. Preoperative transthoracic echocardiography revealed large subaortic ventricular septal defect, 50% aortic over-ride, severe valvular and subvalvular pulmonary stenosis, persistent left superior vena cava (PLSVC), and a large coronary sinus (CS).

A 7-year-old girl was admitted to the hospital for evaluation of requent respiratory tract infection. She gave past history of pneumonia nd was diagnosed with an atrial septal defect (ASD). Physical examnation revealed mild increase in jugular venous pressure and a grade II/VI holosystolic murmur over the left sternal border.

Although persistent left superior vena cava (PLSVC) is rare, it is the most common thoracic venous anomaly. The prevalence of PLSVC is 0.3 % in a general population and 10 % in patients with congenital heart disease. Diagnosis of PLSVC is usually incidental during cardiovascular imaging or surgery.

A case of persistent left with absent right superior vena cava accompanied with atrial septal defect in a six-year-old girl is presented. This rare venous malformation was diagnosed incidentally during surgery when the pericardium was incised. An L-shape cannula was directly inserted into the left superior vena cava for cardiopulmonary bypass.
