ABSTRACT Appendico‐ileal knotting is an extremely rare complication of acute appendicitis, leading to obstruction when the inflammed appendiceal shaft wraps around the adjacent ileal segment and its mesentery. Diagnosis requires high clinical suspicion and urgent surgical exploration.
