ABSTRACT Idiopathic multicentric Castleman disease (TAFRO subtype) can mimic common differentials. High suspicion is required for chronic systemic symptoms and generalized lymphadenopathy. As fine‐needle aspiration is often non‐diagnostic, early excisional biopsy is important.

Ethylene Glycol Poisoning Without Fomepizole in a Regional Australian Intensive Care Unit: The Lactate Gap as the Diagnostic Key and the Consequences of Nasogastric Ethanol Therapy

ABSTRACT A 62‐year‐old man presented comatose with pH 6.88 and no ingestion history. A point‐of‐care lactate above 31 mmol/L with a laboratory lactate of 0.8 mmol/L revealed a lactate gap, diagnosing ethylene glycol poisoning despite a near‐normal osmolar gap. High‐dose continuous renal replacement therapy corrected the acidosis; he survived.

Gastric Emptying in Obesity and Weight‐Loss Interventions: Physiology, Assessment, and Clinical Implications

Obesity is associated with accelerated gastric emptying. Weight‐loss interventions each impose a distinct emptying signature: acceleration after sleeve gastrectomy and gastric bypass, delay after endoscopic sleeve gastroplasty, intragastric balloon, and GLP‐1 receptor agonists.
