Emphysematous gastritis secondary to acute gastric dilatation

Although rare in incidence, it has a fulminating course with a high mortality rate. A case of a 58 year old male with emphysematous gastritis is reported. Initially, he presented with acute gastric dilatation secondary to anorexia/bulimia‐like behaviour.

Acute Gastric Dilatation Revisited
The American Surgeon, 2000First described by S.E. Duplay in 1833, acute gastric dilatation has since been well documented in the literature. Several theories of the pathogenesis of acute gastric dilatation have been postulated. In 1842, Karl Freiherr von Rokitansky described the superior mesenteric artery syndrome, followed by W. Brinton in 1859 with the atonic theory.

Since the pathogenesis of the condition is poorly understood, however, treatment is still empirical. The most effective measure is to intubate the stomach and to remove the accumulated secretions and gases by continuous suction.

Acute Gastric Dilatation after Trauma
The Journal of Trauma: Injury, Infection, and Critical Care, 1987During a 2-year period, 248 consecutive patients were admitted with multiple trauma. Acute gastric dilatation was documented in 67 (27%) patients by review of all admission roentgenograms. There were 51 (76%) males and 16 females. Ages ranged from 2 to 80 years (mean, 29.2 years).
