Spontaneous Acute Arterial Subdural Hematoma

Spontaneous acute arterial subdural hematoma (SDH) is a rare entity caused by hemorrhage from a cortical perisylvian artery without a known precipitant.A 53-year-old man presented with a 2-day history of acute-onset headache and dysphasia. He had a generalized seizure on arrival to hospital and no history of trauma.

• Subdural hematomas were found at operation in 300 patients out of 30,000 hospitalized for recent head injuries. Diagnosis was difficult, since abnormal neurological signs were frequently not of localizing value. Marked dilation of the pupil on one side was associated with ipsilateral lesions in 35 cases, contralateral in 9, and bilateral in 12.
