気道異物は特に乳幼児にとっては生死にかかわることのある救急疾患である。小児例の70%は2歳未満であり,気道異物の多くはピーナッツなどの豆類である。症状は,突然の咳嗽が最も多く,次いで喘鳴である。丁寧な問診と胸部X線は的確な診断に重要である。気道異物を疑った際には全身麻酔下でラリンジアルマスクを用い,摘出前に軟性内視鏡で異物の確認を行う。気道異物の95%は気管あるいは気管支の異物である。摘出術は全身麻酔下で,硬性気管直達鏡や軟性気管支鏡を用いて行う。乳幼児の気管は細く,軟性気管支鏡を用いると換気が不能になるため,主として硬性気管直達鏡を用いている。摘出後,特にピーナッツの場合には術後の管理が重要である。消化器の異物は食習慣や社会の変化とともに変化している。20年以上前は食道異物にはコインが多かったが ...
