The functional outcome of fistula surgery can be quantitated by anal manometry. A closed, water-filled microballoon (0.5×1.0 cm) system was used to measure resting anal pressure and maximal squeeze pressure in 47 patients with anal fistulas at St. Mark's Hospital.

Fistula in ano is a common condition that often recurs despite seemingly adequate surgery, usually because of infection that was missed at surgery. It is now increasingly recognized that preoperative imaging can help identify infection that would have otherwise gone unidentified.

Except for unusual diseases, fistula in ano originates from infection in the anal crypts of Morgagni, forming an abscess which, when it opens, results in a tract leading to the skin surface. A tentative diagnosis can often be made by a careful history followed by local examination.

Abstract It may be stated that fistulas in infants are found predominately in male patients. They are also invariably in lateral situations, taking origin from a lateral crypt, and presenting a secondary opening in a lateral perianal area. The reasons for this peculiarity are perhaps hypothetical.

The present study documents a 15 year experience with anal dilatation in patients with fissure in ano. Patients who were unable to tolerate rectal examination were admitted urgently for anal dilatation. Patients in whom rectal examination and proctoscopy was possible were offered an anal dilator and were reviewed after 4 weeks.

Abstract Three cases of ano rectal pain are described. Although psychiatric illness could be diagnosed in all three the psychopathology varied greatly. The clinical characteristics of the pain suggest that a local organic lesion is the most likely proximate cause despite the inability to establish this by current methods of physical examination.
