BMJ, 2000EDITOR—Shann criticises the use of the APACHE II scoring system as an audit tool for intensive care performance.1 He has two main arguments. Firstly, he says that the system is outdated in that it reflects North American standards in the early 1980s.

Interobserver variability in the use of APACHE II scores
The Lancet, 19992and the simplified acute physiology score (SAPS) are widely used to assess outcome and quality of care in intensive care units. In spite of the widespread use and general acceptance of these scoring systems, there is little information on their reliability and on inter-observer variability in their use.
