This review covers the mechanism and circuit setup of the Controlled Automated Reperfusion of the whoLe body (CARL) system. CARL comprises a pulsatile flow ECMO with a cytoprotective priming solution. Initial results in prolonged OHCA/IHCA show promising neurological‐intact survival rates of 42% in a multicenter analysis.
