目的:膝関節内に発生し,可動域制限を生じた脂肪腫の1例を報告する.症例:37歳,女性.主訴:左膝可動域制限.現病歴:H20年5月頃より誘因なく左膝痛が生じ徐々に膝関節可動域制限も出現した.同年7月当科初診.X線,造影MRIにて異常所見を認めなかった.その後可動域制限が徐々に進行したため精査加療目的で入院した.経過:H21年4月,膝関節鏡施行した.前・後十字靱帯の前後に存在し膝伸展時に大腿骨顆間部とインピンジしていた腫瘤を切除した.病理学的には成熟脂肪細胞と繊維性結合組織を中心とした脂肪腫であった.理学療法を行い疼痛消失し可動域は改善した.考察:関節内脂肪腫は稀な疾患といわれている.過去の文献では,膝関節内腫瘍で可動域制限やロッキングを生じたもののうち約半数が色素性絨毛結節性滑膜炎で ...
