Aspiration pneumonia in the aged.

東京警察病院において過去7年間 (1980~1986年) に嚥下性肺炎と診断された27症例について, 吸引内容, 基礎疾患, 病態, 治療, 予後を分析し, 老年者における嚥下性肺炎の特徴を若年者と対比検討した. 対象例を65歳以上の老年群18例, 65歳未満の若年群9例にわけ, 比較検討した.1. 老年群では胃内容物吸引例が, 若年群に比べ多く認められた. 老年群では胃内容物吸引例が9例と最多であったが, 若年群では食物誤嚥が6例, 胃内容物吸引が3例であった.2. 基礎疾患では, 中枢神経系疾患が多く認められ, 老年群では特に脳血管障害が11例と高率であった.3. 老年群嚥下性肺炎は, 若年群と同様に強い炎症所見を示す傾向を認めた.
