We present a 70-year-old gentleman with chronic cough with haemoptysis found to have left lower zone collapse on imaging. The bronchoscopy showed an endobronchial mass histologically of a non-secreting atypical carcinoid.

Metastatic Atypical Carcinoid Tumor of the Inferior Rectus Muscle
A 74-year-old man who had been treated for pulmonary atypical carcinoid tumor 3 years earlier developed diplopia. Imaging studies showed a well-delineated diffuse enlargement of the right inferior rectus muscle.

Perioperative chemotherapy utility remains uncertain and lacks universal standardization. Platinum/etoposide shows suboptimal efficacy in lung NET (neuroendocrine tumors); oral alkylators (temozolomide) alone or in combination (e.g., with capecitabine), and cabozantinib offer promising disease control.
