The Japanese Journal of Gastroenterological Surgery, 1978 教室で施行した経十二指腸括約筋形成術40例の切除標本について, 剖検32例を対照に臨床病理学的検討を試みた.手術例における組織学的観察から, 総胆管末端部胆汁排出障害の所見としては, 炎症終末像としての線維化のみならず, 亜急性, ならびに急性炎所見, 場合によっては対照例と差のない所見などのみられることが判明した.以上の所見ならびに対照例の検討から, 胆汁排出障害の招来される背景の1因として, 総胆管二指腸内部の生理的狭小化と同部に密に分布する特殊な粘膜ヒダ, とくに前膨大部ヒダを中心とする弁状構造が些細な刺激によって容易に充血, 浮腫などの炎症を招来し, これが同部内腔の狭窄を悪循環的に助長するためと判断された.
