Gastric bezoar (phytobezoar)

A 76-year-old man was referred to the author’s department with melena and anaemia (haemoglobin level 102 g/L). The patient had a medical history of type 2 diabetes mellitus, hypertension and dyslipidaemia. Oesophagogastroduodenoscopy (OGD) showed a gastric bezoar of 5 cm diameter in the fornix (figure 1A) with ulceration of the angulus.

Abdominal symptoms in mentally retarded patients may pose difficult diagnostic problems due to an unreliable history of disease and incooperation of the patients. The case histories are presented of three patients with bezoars in the ileum. Delayed diagnosis and signs of peritonitis necessitated emergency surgery.
