Prospective Trials in Laparoscopic Bile Duct Exploration
Surgical Innovation, 2000Laparoscopic exploration of the common bile duct (CBD) is performed either for the diagnosis or the treatment of CBD stones. Laparoscopic intraoperative cholangiography (IOC) versus ultrasonography (LUS) and laparoscopic versus endoscopic extraction of CBD stones were compared through a review of the prospective, comparative studies, randomized or not,

As pointed out by Way, Admirand, and Dunphy, the true incidence of CBD stones in patients undergoing surgery for gallstones is probably between 12% and 15% in the United States. By using indications essentially identical to those stated below and by performing routine preexploratory cystic duct cholangiography, Way performed CBD explorations in only 21%

Exploration of the Common Bile Duct
1984Exploration of the common bile duct increases the morbidity and mortality in patients undergoing cholecystectomy. Most commonly it is performed to remove ductal calculi often in patients at risk because of jaundice and sepsis or both. Cholangitis may be present and the overall incidence of septicaemia in patients undergoing CBD exploration averages 10%

About 10–20% of patients with cholelithiasis present with choledocholithiasis. 1–2% of patients who undergo a cholecystectomy will present with retained stones postoperatively if intraoperative cholangiography is not done. Open common bile duct (CBD) exploration was the conventional method of stone extraction in the operating room; however, with the ...

Multiple alternatives have largely superseded open common bile duct (CBD) exploration. Endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy provides access to the common duct for extraction of stones and biliary decompression. Laparoscopic transcystic common duct exploration or laparoscopic choledochotomy are alternatives when ...

Indications for exploration of the bile ducts.
International surgery, 1981A series of 477 choledochotomies for suspected choledocholithiasis is presented. The indications for exploration of the common bile duct are reviewed. Absolute indications are: palpable stones in the common bile duct, stones seen on intravenous cholangiography and jaundice during the operation.

Treatment for common bile duct stones (CBDS) has been divided roughly into two different procedures, two-step and single step methods. In the two-step method, LC is performed after endoscopic treatment. The single-step method includes open or laparoscopic surgery in which common bile duct stones are removed during surgery.

Laparoscopic Bile Duct Exploration
2018The majority of bile duct stones are secondary bile duct stones (Chap. 3) that arise in the gallbladder and migrate down the cystic duct. Depending on geographic variation in this incidence of secondary duct stones, trans-cystic exploration successfully clears the bile duct (CBD) in 60–70% of patients; the remainder can be removed by choledochotomy or ...
