The clinical presentation of 12 cases with biliary ascariasis is reported. Ultrasonography was employed to demonstrate worms in the gallbladder and common bile duct preoperatively. Ten cases were operated upon and operative findings were compared to assess the accuracy of sonography in biliary ascariasis.

Biliary ascariasis associated with cholangiocarcinoma
Abdominal Imaging, 1993A 52-year-old man presented with obstructive jaundice. Sonography of the liver disclosed dilatation of intrahepatic ducts in which multiple elongated intraluminal lesions were contained due to biliary ascariasis. A localized mass projecting into the common bile duct and causing its obstruction proved to be a coexistent cholangiocarcinoma.

Sonographic diagnosis of biliary ascariasis
European Journal of Pediatrics, 1990In an 11.5-year-old boy with obstructive jaundice sonography revealed a worm-like structure within the dilated biliary tree. Cross-section showed a bull's eye configuration, whereas longitudinal planes demonstrated an echogenic strip with a central longitudinal anechogenic tube.

Biliary ascariasis, rarely noted in the western literature, is a common condition in other climates. An increase in infestation may occur with travel to these countries. With the changes in biliary function which follow common duct exploration, invasion can occur in the affected individual.
