Bladder exstrophy is a very rare congenital malformation in which the anterior wall of the bladder is absent, and the posterior wall is exposed externally. The differential diagnosis includes omphalocele, gastroschisis, and cloacal exstrophy. Ultrasound and Doppler examinations are the main diagnostic tools.

Abstract A 20-year experience with the management of exstrophy of the bladder is presented. Follow-up data from 27 patients indicate that cystoplasty is a satisfactory operation since half of the older patients developed urinary continence with a normal upper urinary tract.

Embryology of bladder exstrophy
Journal of Pediatric Surgery, 1988A hypothesis in respect to the teratogenesis of bladder exstrophy and its variants is offered. The central feature of this hypothesis is the abnormal persistence of the caudal position of the insertion of the body stalk on the embryo. As a consequence of this, the normal advance and interposition of mesenchymal tissue to the midline becomes impossible.

Management of bladder exstrophy
The Indian Journal of Pediatrics, 2000The exstrophy-epispadias complex is a severe anomaly of the lower urinary tract. While the surgical reconstruction of normal anatomy in patients with this disorder was once considered impossible, modern management has made possible a near-normal reconstruction of the bladder and lower abdominal wall.
