Guarded filtration surgery, is commonly used to control the intraocular pressure (IOP) in glaucomatous patients. Filtration surgery lowers the IOP by creating a fistula between the inner compartments of the eye and the subconjunctival space (i.e., filtering bleb). Treatment of Postfiltration Bleb Leak by Bleb Injection of Autologous Blood
Ophthalmic Surgery, Lasers and Imaging Retina, 1994ABSTRACT We treated a persistent, brisk bleb-leak after trabeculectomy with mitomycin C by bleb injection of autologous blood. Following preparation with povidone iodine, approximately 0.5 cc of whole autologous blood was injected into the bleb at the slit lamp.

Les bulles encapsulees ou «kystes» de la capsule de Tenon sont classiquement associes a une elevation de la pression intraoculaire apres chirurgie filtrante du glaucome. Cependant, cette elevation n'est pas toujours presente.

SummaryBlebs are protrusions of the cell membrane. They are the result of actomyosin contractions of the cortex, which cause either transient detachment of the cell membrane from the actin cortex or a rupture in the actin cortex. Then, cytosol streams out of the cell body and inflates the newly formed bleb. During expansion, which lasts ∼30 s, the bleb

Documentation for techniques of surgical manipulation of a filtering bleb is meager in ophthalmological literature. Of particular interest in this matter is: (1) the management of loss of the filtering bleb after external filtering surgery for glaucoma, (2) management of external fistulization of a filtering bleb, (3) treatment of extension of a ...

Bleb Needling in Encapsulated Filtering Blebs: Evaluation by Optical Coherence Tomography
Ophthalmologica, 2009Purpose: To evaluate the role of anterior segment optical coherence tomography (OCT) in describing the morphology and outcome of encapsulated filtering blebs after bleb needling. Procedures: Prospective assessment of 9 patients – before, 2 days and 6 months after filtering bleb needling with 5-FU – using slit-lamp ...

Cystoid blebs occasionally develop inad vertently in the postoperative cataract inci sion that is well covered by a conjunctival flap. This has been more frequently observed with the use of buried 8-0 virgin silk su tures. Most of these blebs cicatrize within a few months.

Controversy exists regarding the effect of early postoperative bleb leaks on trabeculectomy outcomes. The Flourouracil Filtering Surgery Study found that early wound leaks were strongly associated with failure [29]. However, other authors have not found this association, though the number of leaks in their studies was small [1, 9, 18]. In my experience,
