Clinical features are suggestive of diagnosis if the triad (fever, rash, and eschar) is present. Otherwise clinical diagnosis could be difficult. Pathogenesis of rickettsial eschars: The tache noire of boutonneuse fever
Human Pathology, 1988Prospective investigation of cutaneous lesions of 24 Sicilian patients revealed that 17 were taches noires from patients with a documented diagnosis of boutonneuse fever. Immunofluorescent Rickettsia conorii were demonstrated in 14 of 17 taches noires.

Boutonneuse fever in Portugal: 1995–2000. Data of a state laboratory
European Journal of Epidemiology, 2003From 1995 through 2000, 3239 clinical samples were tested for rickettsiosis at the Center for Vectors and Infectious Diseases Research--National Health Institute in Portugal. Boutonneuse fever due to R. conorii Malish or R. conorii Israel was laboratory confirmed in 374 samples (11.5%). Clinical and epidemiological features are discussed.

In connection with a period of residence in the Mediterranean basin (Portugal, Spain, Sicily) one woman and two men (age: 28, 31 and 40 years) got sick with fever in August and in the midst of October. In addition, they complained about headache in two cases and myalgias, arthralgia and vomiting in one case, respectively.
