Brain abscess in the 1980s

Brain abscess was reviewed in 24 patients admitted to University Hospital, Nottingham over a period of 3 years. Chronic ear infection was the most common predisposing factor, but in 11 patients the focus of infection remained unknown. CT scanning, carried out in all patients, was negative in one patient with clinical signs of meningitis.

Brain abscess in childhood

The author review 10 years' experience in managing brain abscess in childhood: 19 cases were treated in children from 1 to 18 years old. The etiology was rhinogenic in 5 cases, congenital heart disease in 5, hematogenous in 3, traumatic in 3, postoperative in 1, and unknown in 3 cases. Brain abscesses in the young are rare. Only 14 such abscesses have been seen at Texas Children's Hospital since 1968. Most abscesses developed In association with congenital heart disease (5), although sinusitis and mastoiditis were precipitating causes in two patients and one patient, respectively.

Eighteen consecutive cases of brain abscess or cerebritis seen at the Veterans Administration Wadsworth Medical Center (Los Angeles, Calif.) during 1970-1982 were analyzed. Fifteen occurred after the computerized axial tomographic (CT) scanner for diagnosis and follow-up became available in 1976.

Experience with brain abscesses

✓ Eighty-six patients with brain abscesses were seen at the University of Minnesota Hospitals from 1946–1965, and 46 died, a 53% mortality. Multiple and brain stem abscesses accounted for several fatalities, while delayed and missed diagnosis accounted for one third of the deaths.

Clostridial brain abscesses

Ten patients with clostridial abscesses of the brain are presented. Despite the presence of gas within the cerebral hemispheres and Clostridium welchii cultured from the pus obtained, the outcome of all patients managed with burrhole aspiration of the abscess was good.

Summary Three proved and one probable case of brain abscess in infancy are presented. Two of the patients died. With use of the appropriate antibiotic in three cases, plus the intraventricular instillation of Varidase in two instances, the infections were brought under control.
