This case highlights the diagnostic challenge of occult gastric adenocarcinoma presenting with pleuropericardial effusions and tamponade. In unexplained lymphocyte‐predominant effusions, malignancy should be considered, and early VATS‐guided pleural biopsy may be required when noninvasive tests are inconclusive.
