One-hundred twenty-nine patients with carotid artery injuries were analyzed to compare the results of revascularization with those of ligation or occlusion. In patients who present with central neurologic deficit short of coma (Grades 1 to 4), revascularization is clearly the operative method of choice.

The most serious complication that may be attendant upon radical surgery in the neck is compromise of the integrity of the carotid artery system either by elective ligation or by rupture. We present the treatment of 17 extracranial carotid artery aneurysms in 16 patients (1 patient had bilateral lesions).
