Median Arcuate Ligament Syndrome: It Is Not Always Gastritis

A Case of Hepatocellular Carcinoma with Celiac Artery Compression Syndrome.

症例は62歳男性.上腹部痛のため,腹部超音波検査と腹部CT検査をうけ,肝S8に径7cm大の肝腫瘍を指摘された.また,腹部血管造影検査で腹腔動脈起始部に片側性の圧痕様狭窄と,膵アーケード血管の拡張が認められた.正中弓状靱帯による腹腔動脈起始部圧迫症候群を伴う肝細胞癌と診断し,正中弓状靱帯切離術および肝S8亜区域切除術を施行した.病理組織診断は中分化型肝細胞癌であった.術後血管造影で腹腔動脈の狭窄は改善し,総肝動脈や胃十二指腸動脈の血流は順行性となり,膵十二指腸アーケードの拡張は消失していた.術後にTAEが必要となる可能性がある症例は,比較的簡単な手技であるので,正中弓状靱帯切離術を併施し,腹腔動脈起始部の圧迫を解除しておくことが肝要と考え報告した.
