Zero risk for central line-associated bloodstream infections … is this realistic?* [PDF]

open access: yesCritical Care Medicine, 2012
Central line associated bloodstream infections (CLABSI) are one of the most common healthcare associated infections and result in morbidity, prolonged hospitalizations, and increased healthcare costs [1–3]. Over the past decade, and particularly since the Institute of Medicine put hospital-acquired infections in the category of “preventable adverse ...

High Prevalence of Tuberculosis and Serious Bloodstream Infections in Ambulatory Individuals Presenting for Antiretroviral Therapy in Malawi [PDF]

open access: yes, 2011
Background Tuberculosis (TB) and serious bloodstream infections (BSI) may contribute to the high early mortality observed among patients qualifying for antiretroviral therapy (ART) with unexplained weight loss, chronic fever or chronic diarrhea. Methods

Increased Plasma Microbial tDR‐1 in At‐Risk Individuals Is Associated With Decreased Conversion to Clinical Rheumatoid Arthritis and Reduces an In Vitro Macrophage Type 1 Interferon Response

open access: yesArthritis &Rheumatology, EarlyView.
Objective Microbial small RNAs can regulate human genes. Higher plasma concentrations of microbial transfer RNA (tRNA)‐derived RNA‐1 (tDR‐1) were previously associated with lower rheumatoid arthritis (RA) disease activity. This study examined whether tDR‐1 concentrations differ in anticyclic citrullinated peptide‐3 positive (CCP3+) at‐risk individuals (

The synergistic impact of granulocytic myeloid‐derived suppressor cells and innate lymphoid cells in systemic sclerosis

open access: yesArthritis &Rheumatology, Accepted Article.
Objective Systemic sclerosis (SSc) is a chronic autoimmune disorder characterized by immune dysregulation and fibrosis, with myeloid‐derived suppressor cells (MDSCs) emerging as important regulators of immune responses. However, the role of MDSCs in SSc‐associated fibrosis and their interactions with other immune cell populations remain poorly ...

Efficacy, safety and cost‐effectiveness of CAR‐T therapy

open access: yesBritish Journal of Clinical Pharmacology, EarlyView.
CAR T‐cells demonstrate high efficacy in blood cancers, including ALL, MM and DLBCL. Innovations target solid tumours despite challenges such as antigen escape. Combination therapies enhance the delivery and infiltration of CAR T cells. Toxicity, cost and resistance remain major barriers to clinical use.

Best Practice: Implementation of a Central Line Bundle to Reduce Central Line-Associated Bloodstream Infections [PDF]

open access: yes
Central line-associated bloodstream infections (CLABSIs) are among the most frequent health care-associated infections and are reported to be correlated with an increased number of inpatient days, higher cost, and attributable mortality rates (Devrim et ...

Dose‐related propofol resistance in COVID‐19 patients: Altered drug plasma levels, mechanistic insights from neuronal‐derived extracellular vesicles and miRNA analysis

open access: yesBritish Journal of Clinical Pharmacology, EarlyView.
Abstract Aim Critical care for COVID‐19 patients presents unique challenges, particularly in sedation management. Patients demonstrate notable dose‐related resistance to propofol, potentially due to complex pharmacokinetic alterations. Emerging evidence suggests that plasma extracellular vesicle‐associated miRNA represents valuable predictive ...

Systematic review of clinical trials and cohort studies evaluating HIV post‐exposure prophylaxis (PEP) regimens to inform updated Canadian guidelines

open access: yesBritish Journal of Clinical Pharmacology, EarlyView.
Abstract Background We conducted a systematic review of clinical trials and cohort studies on HIV post‐exposure prophylaxis (PEP) regimens to inform updated 2025 Canadian guidelines. Methods We searched Medline and Embase (July 2017–June 2024) for studies reporting on: HIV acquisition, PEP completion, adverse events (AEs) leading to discontinuation and/
