Botulinum toxin for chronic anal fissure
The Lancet, 1994Botulinum toxin can chemically denervate striated muscle. Botulinum toxin A (15 U) was used to treat ten patients with chronic anal fissure by injection in the internal sphincter. In seven patients, the lesion healed at 2 months after treatment; one relapsed at 3 months.

Current Treatment Options in Gastroenterology, 2003
Diagnosis of chronic anal fissure is easy and common in clinical practice. Little is known about the etiology and pathogenesis of this disorder. Current investigations consider anal sphincteric hypertonia and ischemia as primary factors in the appearance and maintenance of this lesion.

Current Treatment Options in Gastroenterology, 1999
Today, chronic anal fissure can be treated and often cured by nonsurgical therapy. Both topical application of glyceryl tri nitrate and intra-sphincteric injection of botulinum toxin appear to be safe and effective in relieving symptoms and in healing the anal fissure in 70% to 90% of patients.

Chronic anal fissure presents as longitudinal, deep ulcer, sometimes coated with smear. Secondary changes can occur in their development, such as hypertrophied anal papillae close to the proximal edge of the fissure at the level of the pectinate line, also pathologically altered crypts (anal cryptitis) or sentinel tags (secondary skin tags) with or ...

Anal Stretch Plus Fissurectomy for Chronic Anal Fissure
Acta Chirurgica Belgica, 2013Anal fissure is a common disease. Usually chronic anal fissures are managed medically. When conservative management fails, surgical treatment should be considered. Lateral internal sphincterotomy has been advocated as the first choice invasive treatment but it has a reported rate of major fecal incontinence of 5%.

A Nonsurgical Approach to Chronic Anal Fissure
Hospital Practice, 1997Both dilatation and sphincterotomy are sometimes followed by impaired continence. Recent experience with topical application of glyceryl trinitrate to relax the internal anal sphincter and cause reversible chemical sphincterotomy has shown promise as a first-line treatment.
