Clonidine has provided a nonaddicting solution to an ancient problem, opiate withdrawal without opiates. Gold and colleagues' initial demonstration that this alpha 2 adrenergic agonist could be used to detoxify opiate addicts provided the first nonaddicting treatment for opiate dependency.

Clonidine Poisoning

Clonidine (Catapres, Boehringer Ingelheim) is a relatively new hypotensive agent with complex pharmacologic actions on the autonomic nervous system. Little is known about its effects and side effects in children. An interesting combination of these were seen in a child who accidentally ingested an unknown quantity of this drug.

Clonidine Overdose

A case of acute intoxication in a 60-yr-old woman who ingested 20 mg of clonidine is presented. The patient showed CNS depression (bradycardia, hypotonia) with systemic hypertension and peripheral vasoconstriction. She was treated with atropine and sodium nitroprusside. There was no recurrence and the patient recovered in 8 days.

Abstract Clonidine’s molecular mechanism of action is agonism at the alpha-2A receptor, a subtype of the alpha-2-adrenergic receptor, found principally within the prefrontal cortex (PFC), where it prevents the secretion of norepinephrine (NE). NE causes the symptoms of withdrawal.
