Some tibial shaft fractures cannot be accurately reduced using closed or percutaneous techniques during an intramedullary nailing procedure. Under these circumstances, a formal open reduction can be performed. Direct exposure of the fracture facilitates accurate reduction but does violate the soft tissue envelope.
