Physiological performance and inflammatory markers as indicators of complications after oesophageal cancer surgery

open access: yesBJS Open, Volume 4, Issue 5, Page 840-846, October 2020., 2020
C‐reactive protein concentration and peak oxygen uptake are collective independent risk factors that can account for over half of major morbidity following oesophagectomy for oesophageal cancer. A composite risk score, the Combined Inflammatory and Physiology Score (CIPS) (range 0–2), predicted major morbidity (17·9 per cent in patients with a CIPS of ...

Impact on postoperative complications of changes in skeletal muscle mass during neoadjuvant chemotherapy for gastro‐oesophageal cancer

open access: yesBJS Open, Volume 4, Issue 5, Page 847-854, October 2020., 2020
Deconditioning during neoadjuvant chemotherapy is a possible parameter for predicting operative outcomes. This retrospective cohort study aimed to assess body composition changes in patients with gastro‐oesophageal cancer during neoadjuvant therapy and investigate their association with postoperative complications. Skeletal muscle index values decrease

Centralizing a national pancreatoduodenectomy service: striking the right balance

open access: yesBJS Open, Volume 4, Issue 5, Page 904-913, October 2020., 2020
A complete 2‐year national cohort of pancreatoduodenectomies was analysed for a volume–outcome effect of postoperative complications, mortality and failure‐to‐rescue. Medium–low‐volume units (fewer than 40 procedures per year) performed on par with a high‐volume unit.

Reintervention or mortality within 90 days of bariatric surgery: population‐based cohort study

open access: yesBJS (British Journal of Surgery), Volume 107, Issue 9, Page 1221-1230, August 2020., 2020
This population‐based study in five Nordic countries, including 49 977 patients, showed that older age and co‐morbidity increased the risk of reintervention or death within 90 days after bariatric surgery. Sex, surgical approach and hospital volume did not appear to influence this risk, but female sex and laparoscopic approach were associated with ...

Conditional survival after neoadjuvant chemoradiotherapy and surgery for oesophageal cancer

open access: yesBJS (British Journal of Surgery), Volume 107, Issue 8, Page 1053-1061, July 2020., 2020
Conditional survival accounts for the time already survived after surgery and may be of additional informative value. A nomogram was constructed that predicts 5‐year survival of patients with oesophageal cancer after neoadjuvant treatment followed by surgery, based on cardiac co‐morbidity, cN category, ypTN stage, chyle leakage and pulmonary ...

Impact of postoperative complications on survival after oesophagectomy for oesophageal cancer

open access: yesBJS Open, Volume 4, Issue 3, Page 405-415, June 2020., 2020
Recent evidence suggests that complications following oesophagectomy may decrease short‐ and long‐term survival. In this study, Clavien–Dindo grade II and IV complications decreased overall and recurrence‐free survival in univariable and multivariable Cox regression analyses.

Association of bacteria in pancreatic fistula fluid with complications after pancreatic surgery

open access: yesBJS Open, Volume 4, Issue 3, Page 432-437, June 2020., 2020
This study showed that, despite the absence of intestinal lumen opening, pancreatic fistulas after distal pancreatectomy frequently contain intestinal bacteria, which markedly worsen the clinical outcome. Understanding the pathogenetic mechanism behind this high prevalence of intestinal bacteria within distal pancreatectomy‐associated pancreatic ...
