Congenital Duodenal Obstruction: An Approach to Diagnosis

53 neonates with congenital duodenal obstruction presented to Our Lady's Hospital for Sick Children over a consecutive 4-year period. Of these 53, 27 presented with intrinsic obstruction, and 26 with extrinsic obstruction. Diagnostic methods and features resulting in surgically proven diagnoses are discussed.

Twenty-nine patients with congenital duodenal obstruction (DO) detected as a "double bubble" (DB) on antenatal ultrasound (US) or diagnosed postnatally were seen at the Women's and Children's Hospital between 1985 and 1994; 24 (83%) had antenatal scans, with 21 (87%) DBs visualised.

Incomplete congenital duodenal obstruction: a diagnostic challenge

A 9-month-old boy was referred with a 6-month history of intermittent bilious vomiting, exacerbated after weaning onto solid food, and faltering growth. He had a diagnosis of trisomy 21, hypothyroidism and repaired congenital heart disease (atrioventricular septal defect and coarctation of the aorta).

Thirty-four congenital duodenal obstructions (19 duodenal atresia, 7 duodenal web, 7 annular pancreas and one duodenal stenosis) were surgically treated in Siriraj Hospital between 1990 and 1999. Eleven per cent of duodenal atresia had no bile-stained vomiting.
