Sternocleidomastoid muscle fibrosis has been recognized for centuries, but its pathogenesis and treatment remains controversial. Pseudotumor of infancy is a firm fibrous mass in the sternocleidomastoid muscle appearing at 2 to 3 weeks of age. Congenital muscular torticollis is less common and appears later in life.

Mandibular asymmetry associated with congenital muscular torticollis

An unusual case involving an adult patient with marked facial and mandibular asymmetry secondary to congenital muscular torticollis is presented. The etiology and pathology of congenital muscular torticollis are reviewed. A team approach was used to correct this deformity. This study of congenital muscular torticollis in infancy is continuing. The data obtained so far may be summarized as follows:1. The tumor in the sternocleidomastoid muscle was discovered at an average age of 3.5 weeks and disappeared at an average age of three and one-half months.

Treatment of congenital muscular torticollis (spasticus) is indicated in cases of neck pain and for cosmetic reasons. Lengthening of the sternocleidomastoid muscle can be achieved by Z-plasty or suture or both muscle bellies. Accessory and greater auricular nerves are at risk during cranial dissection (preparation).
