Intractable pain and cordotomy
BMJ Supportive & Palliative Care, 2020I was interested in the national UK data on percutaneous cervical cordotomy for cancer-related pain.1 They affirm it is effective and early referrals should be considered, rather than analgesics which may be suboptimally effective but also have side effects.

CT-guided Percutaneous Cordotomy
1989Percutaneous cordotomy is a commonly applied and effective procedure among the ablative pain surgeries. As plain X-ray does not permit visualization of the target relative to the electrode the chances of obtaining good results are decreased and the risk of complications are increased. The use of CT has been found to be useful in cordotomy.

Percutaneous stereotaxic cordotomy
Acta Neurochirurgica, 1968A technique of low cervical percutaneous stereotaxic anterolateral cordotomy for relief of intractable pain is described. This is a modification of the method ofGildenberg, Lin, andPolakoff and consists of the use of a surveying telescope developed by us to guide the needle-electrode in the proper angle towards the target.

Pain syndromes that depend on transmission in ascending tracts in the spinal cord, notably the spinothalamic tract, can be treated by ablative procedures, namely cordotomy and myelotomy. This chapter reviews these well-studied, generally reliable, and effective procedures in light of technological improvements and increasing understanding of the ...

The term cordotomy, as applied to the operation of that name, although theoretically meaning an incision anywhere into the spinal cord, is now generally considered to refer to an incision which divides the anterolateral fiber tracts. This procedure was devised by Spiller, in 1912, to relieve intractable pain in the legs or lower part of the body, and ...
