Nonoperative Dilatation of Coronary-Artery Stenosis

In percutaneous transluminal coronary angioplasty, a catheter system is introduced through a systemic artery under local anesthesia to dilate a stenotic artery by controlled inflation of a distensible balloon. Over the past 18 months, we have used this technic in 50 patients.

Supravalvular aortic stenosis (SVAS) is rare in twins. We report monozygotic twins, both of whom had SVAS, coronary ostial stenosis, and peripheral pulmonary arterial stenosis, but no other phenotypic features of Williams syndrome. The ability to accurately measure the effects of a stenosis by perfusion imaging with PET has significant implications for clinical use. This noninvasive technique may facilitate early diagnosis in asymptomatic patients and allow assessment of disease progression. In the meantime, quantitative angiography can be useful.

Iatrogenic coronary artery stenosis following coronary stenting

Catheterization and Cardiovascular Interventions, 1999
We present 6-month follow-up of 435 patients undergoing stent deployment. Forty-four patients were referred because of myocardial ischemia related to the stented artery. In six of these patients (14%), the stented vessel revealed a new proximal lesion separated from the stented portion, which warranted further intervention.

Suture-induced right coronary artery stenosis

An 82-year-old patient developed right heart failure in the days after surgical aortic valve replacement. Coronary CT angiography showed a high-grade stenosis of the mid-right coronary artery. Adjacent suture material seen on noncontrast CT suggested that the lesion was related to surgical closure of the right atrial cannulation site.
