A 35-year-old married man presented with progressive distortion of all the nails of the hands and toes for the past 30 years. Initially, his parents noticed yellowish discoloration and roughness of the thumb nail at the age of 5 years. Since then, the changes have been insidious to involve the other nails.

Darier-White disease (DWD), otherwise known as keratosis follicularis, is a rare disorder of keratinization of the epidermis, mucous membranes, and nails. It is autosomal dominant in transmission. Patients with DWD are prone to frequent superinfection including the rare complication of Kaposi varicelliform eruption.

The most typical clinical symptoms such as diffuse hyperkeratotic papulae usually appear during puberty or early adulthood in seborrhoeic area. Darier-White disease in a Saudi patient associated with systemic involvement.

A 22 year-old Saudi patient is reported who had Darier-White disease (Keratosis follicularis), presenting with the classical cutaneous manifestation of this disease in addition to systemic symptoms, including chronic renal failure, mental retardation, epilepsy, cataract and corneal opacities.
