Using routinely acquired chest CT and admission clinical data, this study developed and temporally validated a model for 90‐day readmission or death after AECOPD hospitalization. Opportunistic CT‐derived body‐composition metrics, especially intermuscular adipose tissue, may add prognostic information beyond conventional clinical scores.
