Delayed splenic rupture in a drug addict

We present the case of a drug addict who was admitted with abdominal pain but gave no history of trauma. He subsequently left the hospital against medical advice, only to be readmitted a few days later with persistent abdominal pain. He was found to have splenic rupture.

A Second Look at Delayed Splenic Rupture

From 1968 to 1976, 314 patients were treated by splenectomy for blunt splenic injuries. Three hundred four of these were operated on within 24 hours. All had typical splenic lacerations with intraperitoneal bleeding from the time of injury. This was true also of seven of the ten operated on after 24 hours.

Delayed splenic rupture: Diagnosed by culdocentesis

Culdocentesis is a valuable asset in the diagnosis of hemoperitoneum in women. A case report of delayed splenic rupture diagnosed by this means emphasizes its usefulness. When compared to paracentesis and peritoneal lavage, culdocentesis is more rapid, theoretically more sensitive, and easier to perform. It is a valuable diagnostic tool.

Due to its location in the abdominal cavity, the spleen is a prime target for damage from blunt injury. Splenic rupture can occur immediately or it can be delayed up to 14 days following injury. A case report is presented involving a patient who sustained a ruptured spleen 2 weeks following an altercation.

Two cases of secondary haemorrhage into subcapsular haematomas with delayed splenic rupture are presented. Ultrasound and computed tomography were essential in the diagnosis. In one of the cases a large splenic laceration was diagnosed by these two techniques 23 days following trauma.

The spleen is the most commonly injured organ in blunt abdominal trauma and is also frequently injured as a result of penetrating trauma to the left torso or upper abdomen. Most splenic injuries manifest at the moment of injury with symptoms of acute intraperitoneal hemorrhage and shock.

Abstract A case is reported of delayed splenic rupture occurring four days after the original trauma of an automobile accident. A study of fifty-eight such cases indicates that approximately 50 per cent occur during the first week after injury and about 25 per cent additional during the first half of the second week. A 46-year-old man was assaulted with a club to the face, chest and abdomen. On admission there were no signs of circulatory shock. The abdominal examination was unremarkable. During his stay in hospital he had light diets. He was managed symptomatically and discharged 5 days after the incident.
