ABSTRACT Gastrointestinal histoplasmosis is an under‐recognized manifestation of disseminated Histoplasma capsulatum infection. Its ileocecal predilection closely mimics intestinal tuberculosis (TB) and Crohn's disease, posing serious diagnostic challenges, particularly in TB‐endemic regions.
