Extracellular Vesicles Modulation by an Adiponectin Receptor Agonist Provides Cardioprotection for Myocardial Ischemic Injury

This study demonstrates that ALY688, a drug mimicking the heart‐protective hormone adiponectin, reduces myocardial ischemia injury. ALY688 increases the production of extracellular vesicles, which carry protective cargo including adiponectin itself.

ABSTRACT Neonatal limb gangrene with auto‐amputation is rare and often idiopathic. Early recognition, prompt referral, and thorough etiologic workup—despite resource limitations—are critical. Wet gangrene requires broad‐spectrum antibiotics, wound care, and timely surgical amputation to preserve growth plates and allow future prosthesis fitting.

Severe Lactic Acidosis in Decompensated Cirrhosis Despite Nondiagnostic Imaging

ABSTRACT Lactic acidosis is a strong predictor of mortality in cirrhosis, reflecting both impaired hepatic clearance and systemic tissue hypoxia. We describe a 38‐year‐old man with decompensated alcohol‐associated cirrhosis who developed severe lactic acidosis despite stable hemodynamics and initially nondiagnostic vascular imaging.
