A case of double pylorus

Doubling of the pyloric channel is one of the rare anomalies of the intestinal tract. There have been 23 cases of double pylorus that we have been able to find in the literature. As the etiologic aspects of double pylorus the majorities of the cases were thought to be acquired lesions from ulcer disease except three cases.

Double Pylorus and Esophageal Achalasia

29 cases of double pylorus have been reported so far. It is thought that in 85% of the cases it has been due to a fistulous gastro-duodenal ulcer and the remaining 15% would be of a congenital origin. In the case we have reported the double pylorus is believed to be a congenital anomaly associated with a mega-esophagus of similar etiology.

Double pylorus (DP) or duplication of the pylorus is an uncommon condition that is either congenital or acquired. Acquired double pylorus (DP) results from a peptic ulcer eroding through and creating a fistula between the duodenal bulb and the distal stomach.

We report a 55-year-old man presenting with postprandial epigastric pain and vomiting. Barium meal study suggested two openings from the stomach to the duodenum. Endoscopy revealed double pylorus with chronic duodenal ulcer, suggesting the second opening as an acquired one.

Smith and Tuttle described in 1969 for the first time a double pylorus; since then this anomaly has been found repeatedly. The pathogenesis is still discussed; the anomaly may be congenital or acquired. A case report is given of a patient with double pylorric ulcer.
