Journal of Clinical Pharmacy and Therapeutics, 1988The incidence of drug administration errors was prospectively evaluated from 1098 drug administrations in 97 paediatric patients. The total incidence of errors was 12.9% in a previous study. An educational programme (lectures and a book on guidelines for drug administration) was offered to nurses in an attempt to decrease this error rate.

[Drug errors during anesthesia].
Masui. The Japanese journal of anesthesiology, 2008We report drug errors during anesthesia. The data were retrieved from the incident reports in a period of June 2000 to June 2003 at the Department of Anesthesiology, Fukuoka University Hospital. Drug errors occurred in 18 (0.12%) of 15,271 patients who were managed by anesthesiologists during the period.
