Dystonia is a movement disorder with a complex and not fully understood pathophysiology. Its better understanding would enable more focused treatment for the disorder. In this review, we provide an overview of recent studies of the pathophysiology of primary and secondary dystonia, with an emphasis on functional brain imaging.

We studied 14 patients who had a combination of idiopathic torsion dystonia and myoclonic jerks. In many cases, the same muscles were involved in both the myoclonus and the dystonia. This made observation of the crucial dystonic postures difficult and led to misdiagnosis of other dyskinesias.

This article provides a summary of the state of the art in the diagnosis, classification, etiologies, and treatment of dystonia.Although many different clinical manifestations of dystonia have been recognized for decades, it is only in the past 5 years that a broadly accepted approach has emerged for classifying them into specific subgroups.

In most cases the cause for dystonia is unknown. In a small number of patients, trauma precedes the onset of the dystonia. Significant head trauma is a well-recognized precipitating factor in dystonia. However, it has become increasingly recognized that peripheral trauma can also result in dystonia.
