After ischemic stroke, cholesterol accumulation in microglia is accompanied by the emergence of a pro‐inflammatory IFITM3+ subset with aberrant esterification and lipid droplet buildup. This metabolic stress is linked to mitochondrial dysfunction, STING activation, sustained neuroinflammation, and white matter injury.
