Teaching Tracheal Intubation to Emergency Medical Technicians

救急救命士の気管挿管が認められ, 病院での気管挿管実習が始まった. 大阪労災病院で実習を修了した救急救命士12名の問題点として, 麻酔器を用いたマスク換気が十分にできず, 人形を用いた事前訓練による悪い癖がついているため喉頭鏡操作がうまくできないことがある. 気管挿管成功率は92±6%であり, 個人間で技術にばらつきがみられるが, 年齢と成功率には有意な相関はなかった. 合併症は咽頭痛が12.5%, 嗄声が19.1%にみられた. 指導医は, 救急救命士が気管挿管を行うことの意義をよく理解し, 気管挿管実習とは総合的な気道管理実習の一部であると認識しなければならない. まずマスク換気がしっかりできること, そして確実かつ合併症を起こさない挿管技術を指導することが重要である.
