症例は31歳の女性. 20年来SLE,ループス腎炎にてステロイド治療を受けていた.臀部痛,仙骨部皮疹の出現後約2週間して頭痛,発熱,意識障害をきたし緊急入院.頭部MRIで脳底部を中心とした出血を伴う異常信号域を認め,髄液VZV-DNAPCR陽性より帯状庖疹ヘルペスによる脳炎と診断した.帯状庖疹は高率に髄膜炎を伴うが,免疫不全宿主では脳炎への進展など重症化することがあり注意を要する.
