Stroke is the second leading cause of death worldwide. One of the main causes of stroke is carotid artery stenosis. Stenosis with atherosclerosis in the carotid artery can cause stroke by hemodynamic ischemia or artery to artery embolism. A most common surgical intervention for carotid artery stenosis is carotid endarterectomy (CEA).

Surgical Clinics of North America, 1980
Thousands of carotid endarterectomies are now performed each year. In properly selected patients in the hands of competent surgeons, the mortality and morbidity rate should not exceed 2 per cent. Although the long-term results are good, this procedure is prophylactic, and a patient who has already sustained a fixed neurologic deficit will rarely be ...

Although controversies still exist, recently reported trials have confirmed the efficacy of carotid endarterectomy and more clearly elucidated the appropriate indications for surgical therapy. The efforts to optimize outcomes for patients with carotid artery disease have expanded to the asymptomatic patient, the patient suffering from stroke, and the ...

During the last 20 years carotid endarterectomy has become an important method of treatment for selected patients with arteriosclerotic cerebrovascular disease. Improvement in selection of patients for operation, in anesthetic management, and in surgical techniques has resulted in a decline in complications associated with the operation.

Extraluminal Carotid Endarterectomy
Archives of Surgery, 1972Twenty-five patients have undergone carotid endarterectomy by extraluminal dissection of the plaque. An incision is made into the media of the common and internal carotid arteries onto, but not through, the plaque to be excised. The plaque can then be dissected from the carotid bifurcation prior to entrance into the lumen of the carotid artery.
