A post‐eparterial bronchus is a rare anatomical variant that can predispose to infection due to secretion stasis, especially in susceptible patients. Navigating this challenging airway can be achieved by the combination of an ultrathin bronchoscope for optimal access and mobile cone beam CT for definitive ‘tool‐in‐lesion’ confirmation.
