The authors present ten years' experience with endomyocardiac biopsy of the heart (EMB) using the percutaneous, endovasal catheterization needle technique. EMB was performed in 314 patients, a total of 1362 biopsies, and for evaluation 5564 specimens of cardiac tissue were taken.

Short-term survival of cardiac allografts has increased markedly because of surveillance by protocol endomyocardial biopsies and improvements in immunosuppression, which have largely prevented graft loss from acute rejection. Currently, 85% of allograft recipients survive for 1 year, and 70% can expect to survive 5 years.

Endomyocardial biopsy is an accepted, useful invasive tool for the analysis of human endomyocardium at the cellular and subcellular levels. It is applicable in the evaluation of specific diseases including cardiac allograft rejection, myocarditis, anthracycline cardiotoxicity, and infiltrative cardiomyopathies.
