Individualized Treatment in Distal and Medium Vessel Occlusion Stroke Using a Validated Explainable Counterfactual Treatment Estimation Model

open access: yesAnnals of Neurology, Volume 99, Issue 5, Page 1198-1209, May 2026.
Objective The optimal treatment for distal medium vessel occlusion (DMVO) stroke remains uncertain, and evidence comparing endovascular therapy (EVT) with medical management (MM) is limited. We aimed to develop and validate a predictive modeling tool to assess individual treatment benefit in DMVO stroke using explainable counterfactual treatment ...

Intracranial Hypotension Following Spinal Manipulation: A Case Report and Scoping Review of the Literature

open access: yesBrain and Behavior, Volume 16, Issue 5, May 2026.
Abstract Purpose Spinal manipulative therapies, including chiropractic and osteopathic maneuvers, are widely practiced for musculoskeletal complaints. However, rare but serious complications such as cerebrospinal fluid (CSF) leak with subsequent intracranial hypotension (IH) have been described.

Chronic Subdural Hematoma in a Patient With Long‐Standing Syphilis

open access: yesClinical Case Reports, Volume 14, Issue 5, May 2026.
ABSTRACT Chronic subdural hematoma can rarely occur in younger adults without trauma or coagulopathy. Comprehensive imaging to exclude vascular malformations, combined with burr hole evacuation and middle meningeal artery embolization, can achieve excellent outcomes.

Admission Shock Index Is an Independent Predictor of In‐Hospital All‐Cause Mortality in Patients With Acute Aortic Dissection and Intramural Hematoma

open access: yesClinical Cardiology, Volume 49, Issue 5, May 2026.
An admission shock index (SI) ≥ 0.6 is a simple, rapid predictor of higher in‐hospital mortality in patients with acute aortic dissection and intramural hematoma. This early risk stratification tool is especially valuable for identifying high‐risk patients managed with conservative treatment.

HBO‐PC Reprograms Neuroimmune Metabolism Through Disruption of the LRG1‐HIF‐1α‐IL‐6‐STAT3 Amplification Loop Attenuates Pyroptosis and Ischemia–Reperfusion Injury

open access: yesCNS Neuroscience &Therapeutics, Volume 32, Issue 5, May 2026.
A mouse CIRI model was generated by middle cerebral artery occlusion. HBO‐PC and LRG1 siRNA knockdown were applied. Neurological function and molecular changes were evaluated. We confirmed that HBO‐PC reprograms neuroimmune metabolism through disruption of the LRG1‐HIF‐1α‐IL‐6‐STAT3 amplification Loop attenuates pyroptosis and ischemia–reperfusion ...

Sestrin2 Mitigates Neuronal Ferroptosis Following Subarachnoid Hemorrhage via Orchestration of the AMPK/PGC1α/Nrf2 Signaling Axis

open access: yesCNS Neuroscience &Therapeutics, Volume 32, Issue 5, May 2026.
The schematic summarizes SESN2's neuroprotective actions after subarachnoid hemorrhage. SESN2 activates an AMPK/PGC1α/Nrf2 signaling cascade that reduces oxidative stress, suppresses neuronal ferroptosis, and improves functional outcomes; these effects were validated by genetic knockdown and pharmacologic inhibition.
