Spinal Epidural Hematoma and Epidural Anesthesia
THE JOURNAL OF JAPAN SOCIETY FOR CLINICAL ANESTHESIA, 2012 硬膜外血腫の発生例の報告は最近増えてきている．以前考えられていた発生率よりも高く，最近では3,100から40,000例に1例程度と推定されている．硬膜外血腫を防ぐためには，カテーテル留置は避けること，挿入時の長さは5cm以内，硬膜外麻酔の施行に固執しないこと，抗凝固療法を適切に行うこと，などがあげられている．また硬膜外術後鎮痛の施行時には，十分なモニタリングを行い，施設での予防管理手順を作成することが大事である．また周術期の抗凝固薬や抗血小板薬の使用と神経ブロックの施行との関係のガイドラインにも習熟しておく必要がある．硬膜外麻酔の有用性を生かすためにも安全な施行方法を知るべきであろう．

THE JOURNAL OF JAPAN SOCIETY FOR CLINICAL ANESTHESIA, 2005 硬膜外腔圧と硬膜外麻酔の広がりとの関係について, これまでに報告された研究を概説した. 硬膜外腔に薬液を注入したときに生ずる硬膜外腔の圧変化を椎間孔の開存性だけで説明することはできない. 硬膜外腔の圧変化を頭蓋腔を含めた圧緩衝システムの応答としてとらえると, 硬膜外腔のコンプライアンスが大きく, 抵抗が小さいほど, 薬液注入によって硬膜外腔の潜在空間が物理的によく広がると考えられる.
