This is the first reported case in the literature of an extra extensor muscle and tendon to the right thumb. It was discovered during routine dissection of a male cadaver. The muscle originated from the dorsal surface of the lower third of the ulna immediately proximal to the origin of the extensor indicis proprius.

Extensor Indicis Proprius Opponensplasty
Journal of Hand Surgery, 1991An analysis of 39 patients (40 hands) who underwent an extensor indicis proprius opponensplasty was carried out. Of these, 29 hands had simultaneous adjuvant surgery to correct other deformities. The mean follow-up period was 33.8 months. Excellent or good results were seen in 87.5%, fair in 10% and poor in 2.4% hands.

Hand Clinics, 1989Rheumatoid tendon ruptures occur primarily by the effect of the tenosynovium on the tendon or secondarily by the effect of the tenosynovium on the bone. An understanding of these complications combined with a knowledge of the anatomy helps to explain the pathology and suggest possible treatments.

The distinguishing feature of extensor mechanism malalignment is passive patellar hypermobility at 30 degrees of flexion, which yields apprehension.
